Short-Term Therapy Success

Ms. E. admitted to Shady Lawn following hospitalization due to a significant fall at home. She was having increased weakness and unable to safely return home. After her hospitalization, Ms. E was debilitated and needed rehabilitation services including PT and OT services with focus on core and extremity strengthening, balance training, ambulation, activities of daily living, and activity tolerance. Prior to her hospitalization she was independent with activities of daily living and ambulated with rollator unlimited distances. Ms. E was walking ten feet on admission with moderate assistance with a walker, standing supported for activities of daily living and took part in activity tolerance for 3-5 minutes, transferred with moderate assistance, and completed most activities of daily living with moderate assistance. After working with therapy, Ms. E was walking seventy feet with supervision, improved her standing balance and activity tolerance to 15 minutes, and activities of daily living with supervision. Ms. E was able to discharge home with her spouse.
