Short-Term Therapy Success

Ms. D was admitted to Shady Lawn following hospitalization due to a significant fall at ALF. She was having increased weakness with a functional decline and was unable to safely return to the ALF. After her hospitalization, Ms. D became debilitated and needed extensive rehabilitation services including PT and OT services, with focus on core and extremity strengthening, postural training, balance training, ambulation activities of daily living and activity tolerance. Prior to her hospitalization, she was supervision with activities of daily living and ambulated fifty feet with rolling walker. Ms. D was unable to walk on admission, unable to stand, transferred with moderate assist and completed most activities of daily living with moderate assistance. After working hard in therapy, Ms. D was able to return to walking fifty feet using a rolling walker with supervision, improved her standing balance, transferred with supervision, and took part in activities of daily living with supervision. Ms. D was able to discharge back to ALF after receiving PT and OT.
