Short-Term Therapy Success: David H.
David H. was admitted to Shady Lawn following hospitalization due to increased weakness, altered mental status, and unable to safely return home. After his hospitalization, David was debilitated and needed extensive rehabilitation services including PT, OT, and ST services with a focus on core and extremity strengthening, posture training, balance training, ambulation, activities of daily living, dysphagia, and cognitive training. Prior to his hospitalization, he was supervised with activities of daily living, ambulated with a cane, and able to communicate wants and needs. David was walking 10 feet on admission with a rolling walker with moderate assistance, standing unsupported for activities of daily living for 1-3 minutes, and required maximum assistance for safety awareness due to loss of balance and visual deficits. After working hard in therapy, David was walking 250 feet with supervision with a rollator, improved his standing balance to 15 minutes, and participated in mock scenarios for home management tasks. He improved his cognition and participated in higher-level vocational tasks. David was able to be discharged home after receiving OT, PT, and ST, David was motivated to return home and worked hard in his therapy.
