Bobby G. was admitted to Shady Lawn following hospitalization due to having multiple falls at home with resultant hospitalization. He was having increased weakness and unable to safely return home. After his hospitalization, Bobbys debilitation needed extensive rehabilitation services including PT and OT services with focus on core and extremity strengthening, postural training, balance training, ambulation, activities of daily living, and activity tolerance. Prior to his hospitalization he was independent with activities of daily living and ambulated with a walker. Bobby was walking fifty feet on admission with walker with moderate assistance, standing supported for activities of daily living and activity tolerance for 5 minutes, transferred with moderate assistance, and completed most ADLs with moderate assistance. After working hard in therapy, Bobby was walking two hundred feet with supervision with a walker, improved his standing balance to 15 minutes, activity tolerance to 30 minutes, and ADLs with independence. Bobby discharged home with his spouse after receiving PT and OT. Bobby motivated himself to return home and worked hard in his therapy.
