Short-Term Therapy Success Stories

C. P. was admitted to Shady Lawn following hospitalization due to short illness. C. P. was having multiple falls at home and increased weakness prior to his hospitalization. After his hospitalization, his debilitation needed rehab services including occupational and physical therapy 5 days a week. His therapies included core and all extremity strengthening exercise, neuromuscular facilitation, posture training, balance training, ambulation, and ADL’s. Upon admission, C. P. was able to walk seventy-five feet with multiple rest breaks and loss of balance. He was transferring, dressing, bathing, and completing hygiene with minimal assistance. At discharge, he was walking seventy-five feet independently with a rollator. His transfers improved from minimal assistance to independent. C. P. was also doing activities of daily  living, bathing, hygiene tasks, and transferring independently. His goal was to be discharged home safely. C. P. was a pleasure to work with. His hard work and motivation paid off and he discharged back into the community at his prior level of function.
