Carroll admitted to Shady Lawn following hospitalization due to a significant fall at home. He was having increased weakness and was unable to safely return home to live independently. After his hospitalization, Carroll’s debilitation needed extensive rehabilitation services including PT and OT with focus on core and extremity strengthening, postural training, balance training, ambulation, activities of daily living, and activity tolerance. Prior to his hospitalization he was independent with activities of daily living and ambulated unlimited distances with no assistive device. Carroll was walking 75 feet on admission with walker and moderate assistance, standing supported for activities of daily living and activity tolerance for 5 minutes, transferred with moderate assistance, and completed most activities of daily living with moderate assistance. After working hard in therapy, Carroll was walking unlimited distances over 500 feet with a rollator, improved his standing balance to 15 minutes, activity tolerance to 30 minutes, and activities of daily living with independence. Carroll was able to discharge home independently after receiving PT and OT. Carroll returned home after working hard in therapy.
